In apache configuration file (httpd.conf), make sure the default for DirectoryIndex includes index.php. In Centos/Red Hat edit /etc/httpd/conf/httpd.conf. In Ubuntu or Debian I think you need to edit /etc/apache2/httpd.conf but don't rely on this second one as it's for my own reference.

Make sure you restart apache. In Cntos it is /sbin/service httpd restart and in Ubuntu it is /etc/init.d/apache2 restart

If this doesn't solve the problem make sure the tmp directory exists and check the error log.
